RESUMO

ObjectiveTo observe the effect of Shenling Baizhusan on the intestinal inflammatory reaction in the rat model of Crohn's disease (CD) and study its relationship with p38 mitogen-activated protein kinase (MAPK) signaling pathway, so as to provide an experimental and theoretical basis for the clinical application of this prescription. MethodA total of 72 SD rats (36 males and 36 females) were randomized into a normal group (n=12) and a modeling group (n=60). The rats in the modeling group were treated with 2,4,6-trinitrobenzene sulfonic acid (TNBS, 3 mL·kg-1) and then randomized into model, mesalazine (0.21 g·kg-1·d-1), and low-, medium-, and high-dose (5.88, 11.76, 23.59 g·kg-1·d-1, respectively) Shenling Baizhusan groups. The rats in the drug intervention groups were administrated with corresponding agents by gavage for 14 days, and those in the normal and model groups with an equal volume of distilled water. The disease activity index (DAI) score of inflammatory bowel disease (IBD) and the colon mucosal damage index (CMDI) score of rats in each group were assessed after gavage. Hematoxylin-eosin (HE) staining was used to observe the histopathological changes in the colon, and enzyme-linked immunosorbent assay (ELISA) to measure the levels of tumor necrosis factor alpha (TNF-α), interleukin-1 (IL-1), and interleukin-6 (IL-6) in the serum. Western blotting was employed to determine the protein levels of p38 MAPK, phosphorylated p38 MAPK (p-p38 MAPK), p65 nuclear factor (NF)-κB, and phosphorylated-p65 NF-κB (p-NF-κB p65) in the colon tissue. Quantitative real-time polymerase chain reaction was conducted to determine the miRNA levels of p38 MAPK and NF-κB p65 in the colon tissue. ResultThe model group had higher DAI and CMDI scores than the normal group (P<0.01) and showed damaged epithelial cells in the colon mucosa, disarrangement of glands, damaged simple tubular glands, local necrosis, infiltration of a large number of inflammatory cells and lymphocytes in each layer, and presence of ulceration. Compared with the normal group, the model group showed elevated levels of TNF-α, IL-1, and IL-6 in the serum (P<0.01) and up-regulated protein levels of p-p38 MAPK and p-NF-κB p65 and miRNA level of p38 MAPK in the colon tissue (P<0.01). Compared with the model group, mesalazine and high- and medium-dose Shenling Baizhusan decreased the DAI and CMDI scores (P<0.05, P<0.01), repaired the mucosal epithelium of the colon tissue, increased the glands and goblet cells, lowered the levels of TNF-α, IL-1, and IL-6 in the serum (P<0.05, P<0.01), and down-regulated the protein levels of p-p38 MAPK and p-NF-κB p65 and the miRNA level of p38 MAP in the colon mucosa (P<0.01, P<0.05). ConclusionShenling Baizhusan can reduce intestinal inflammation of CD rats and promote the repair of colon mucosa by down-regulating the protein levels of p-p38 MAPK and pNF-κB p65 and the miRNA level of p38 MAPK to inhibit the p38 MAPK pathway.

RESUMO

Objective:To evaluate the long-term efficacy of endoscopic transluminal drainage(ETD) for acute pancreatitis complicated with walled-off necrosis (WON) or pancreatic pseudocyst (PPC).Methods:A total of 79 patients who were diagnosed as having WON or PPC by abdominal CT or ultrasound and treated with ETD in Nanjing Drum Tower Hospital were enrolled. Past medical records and follow-up by phone call after discharge were analyzed for long-term outcomes including endocrine and exocrine functions and long-term quality of life. Results:A total of 50 patients were enrolled, including 31 patients with infected WON/PPC and 19 patients with uninfected WON/PPC. Seventeen patients (54.84%) in the infected WON/PPC group and 11 patients (57.89%) in the uninfected WON/PPC group lost 5% or more of their weight. There were no significant differences in the proportion of cases of weight loss of 5% or more ( P=0.833), or the weight loss between the two groups (12.59±8.89 kg VS 10.91±2.47 kg, P=0.522). Only one patient in the infected WON/PPC group had chronic abdominal pain. There was no significant difference in the Izbicki score between the two groups (23.79±6.74 VS 22.03±3.21, P=0.295). None of the patients developed steatorrhea after discharge. Five patients (16.67%, 5/30) in the infected WON/PPC group and 6 patients (40.00%, 6/15) in the uninfected WON/PPC group developed endocrine insufficiency with no significant difference ( P=0.140). Greater risk of secondary diabetes resulted from higher low-density lipoprotein cholesterol ( HR=1.9, 95% CI: 1.0-3.4, P=0.044)and triglycerides ( HR=1.2, 95% CI: 1.0-1.3, P =0.029). Conclusion:ETD is safe and effective for WON and PPC. But there is possibility that patients develop secondary diabetes. Additionally, greater risk of secondary diabetes results from higher low-density lipoprotein cholesterol and triglycerides.

RESUMO

【Objective】 To study how blood donation intention among college students was influenced by publicity information (framework effect), individual characteristic (altruistic personality) and micro-system of the social ecological systems(organization trust) through the viewpoints of social psychology, sociology and behavioral economics, aiming at providing new strategies for donors recruitment in colleges. 【Methods】 A total of 1 225 questionnaires were distributed to four universities in Xiamen and Quanzhou, Fujian Province from June 2021 to December 2021, with blood donation publicity information included, to investigate the blood donation intention, altruistic personality, organization trust and demographic characteristics. 【Results】 A total of 99.18%(1 215/1 225)of the collected questionnaires were valid. The average score of blood donation intention was (76.40 ± 24.31) (the range of value was 0-100). Multiple linear regression showed that altruistic personality and organization trust were positively associated with donation intention (P<0.05). Altruistic personality and organization trust had a significant interaction; with the increase of organization trust, the correlation between altruistic personality and donation intention would decrease (P<0.05). In addition, the publicity information had significant frame effect on blood donation intention when other variables in the regression model were controlled (P<0.05). Compared to the negative framework, the positive framework can promote the donation intention better. 【Conclusion】 When recruiting potential blood donors among college students, positive image of blood centers is conductive to promote organization trust therefore promote donation intention. The effective use of framework effect to boost blood donation intention might improve the efficiency of donor recruitment to a certain extent.

RESUMO

Objective:To investigate the clinicopathological features of gastric adenocarcinoma of fundic gland type (GA-FG).Methods:A total of 12 patients, including 7 cases treated with endoscopic submucosal dissection (ESD), were diagnosed as having GA-FG in Nanjing Drum Tower Hospital from January 2018 to August 2019. Morphological changes were analyzed by reviewing endoscopic and pathological results. Patients were followed up after definitive diagnosis. Results:The clinical symptoms of patients with GA-FG were nonspecific. No Helicobacter pylori infection was identified. The lesions were found in the non-atrophic gastric mucosa of the upper 1/3 portion in 10 cases and middle 1/3 portion in 2 cases. Endoscopically, the most common features were whitish color (9 cases), and all lesions diameter≤1 cm. Their macroscopic types were classified as 0-Ⅰ (2 cases), 0-Ⅱa (9 cases) and 0-Ⅱc (1 case) respectively. All lesions had sharp boundary, with branching dilated blood vessels on the surface. Five in 7 cases who were treated with ESD showed submucosal invasion. Immunohistochemically, 9 cases were classified as the chief cell type , 3 as the mixed type, 11 MUC6 positive, 4 MUC5AC positive, 2 MUC2 positive, and 3 CD10 positive. P53 was detected in all 12 cases, and 9 cases had low Ki-67 staining index (<10%). The mean time of follow-up was 11 months, and 11 patients survived. Conclusion:GA-FG should be taken into consideration when the polyps are found in the upper part of the stomach, with whitish color, and branch dilated blood vessels on the surface. Excellent clinical outcomes can be achieved for GA-FG patients with ESD.

RESUMO

Objective:To investigate the effect of transforming growth factor (TGF- β) signal in muscle fiber itself during inflammation/immunity response on intramuscular inflammation. Methods:Sixteen wild C57BL/6 mice (wild group) and sixteen mice with skeletal muscle-specific deficiency of T βRⅡ (knock-out group) between 4-8 weeks of age were selected for this study. Acute muscle injury in mice was induced by injection of myotoxin cardiotoxin (CTX) into gastrocnemius. The differences in intramuscular inflammation were compared between the wild and knock-out groups on 0, 4, 7 and 10 d after CTX injection by observing exudation of mononuclear phagocytes, macrophages, M1 type macrophages, CD4 +T cells and helpers T cells (Th1, 2&17). Two newborn C57BL/6 wild mice and 2 SM TGF- βr2-/- knock-out mice were selected to culture primary myoblasts in vitro which were divided into 2 groups: an interferon group subjected to interferon simulation and a control group subjected to addition of an equal amount of solvent. The differences in expression of IL-6, IL-10, MCP-1, MIP-1α, H-2K b, H2-Ea, Toll-like receptor (TLR)3 and TLR7 were compared between the interferon and control groups, as well as between the wild and knock-out groups. Results:On 4&7 d after CTX injection, the ratios of mononuclear/macrophage (75.73%±3.62%, 45.27%± 2.32%), macrophages (38.67%±2.76%, 24.87%±2.19%), M1 macrophages (43.21%±0.11%, 30.43%±2.19%), CD4 +T cells (20.13%±1.62%, 5.67%±0.32%) in the muscle tissue from the knock-out mice were significantly higher than those from the wild mice (58.52%±2.43%, 29.21%±2.45%; 20.63%±2.32%, 16.23%±1.25%; 24.98%±0.35%, 14.23%±1.69%; 10.70%±0.43%, 2.50%±0.45%), with a majority of Th1&Th17 ( P<0.05). In vitro results showed that the levels of IL-6, MCP-1, MIP-1α, H-2K b, H2-Ea and TLR3 were significantly upregulated in the interferon group compared with the control group and that such upregulation in the nock-out mice was more significant than in the wild mice ( P<0.05). Conclusions:Endogenous TGF- β signal activation plays a role in the functional recovery after muscle trauma, because it is involved in the regulation of immune behavior of muscle fibers, thus affecting intramuscular inflammation and muscle regeneration.

RESUMO

BackgroundChest CT had high sensitivity in diagnosing novel coronavirus pneumonia (NCP) at early stage, giving it an advantage over nucleic acid detection in time of crisis. Deep learning was reported to discover intricate structures from clinical images and achieve expert-level performance in medical image analysis. To develop and validate an integrated deep learning framework on chest CT images for auto-detection of NCP, particularly focusing on differentiating NCP from influenza pneumonia (IP). Methods35 confirmed NCP cases were consecutively enrolled as training set from 1138 suspected patients in three NCP designated hospitals together with 361 confirmed viral pneumonia patients from center one including 156 IP patients, from May, 2015 to February, 2020. The external validation set enrolled 57 NCP patients and 50 IP patients from eight centers. Results96.6% of NCP lesions were larger than 1 cm and 76.8% were with intensity below -500 Hu, indicating less consolidation than IP lesions which had nodules ranging 5-10 mm. The classification schemes accurately distinguished NCP and IP lesions with area under the receiver operating characteristic curve (AUC) above 0.93. The Trinary scheme was more device-independent and consistent with specialists than the Plain scheme, which achieved a F1 score of 0.847, higher than the Plain scheme (0.774), specialists (0.785) and residents (0.644). ConclusionsOur study potentially provides an accurate early diagnosis tool on chest CT for NCP with high transferability, and shows high efficiency in differentiating NCP and IP, helping to reduce misdiagnosis and contain the pandemic transmission.

RESUMO

Objective:To study the different clinical features of early gastric cardia carcinoma (EGCC) patients with synchronous tumor, and identify the risk factors and prognosis of the occurence of synchronous tumor after endoscopic resection. Methods:A retrospective study was made on the data of 499 patients (512 lesions) who underwent cardiac endoscopic submucosal dissection (ESD) and were pathologically confirmed as EGCC in Nanjing Drum Tower Hospital from January 2011 to June 2018. The patients were divided into synchronous tumor group (47 patients with 50 lesions) and non-synchronous tumor group (452 patients with 462 lesions). Univariate and multivariate logistic regression analysis were performed to identify independent risk factors for the occurrence of synchronous tumor. Kaplan-Meier method was conducted to prognosis. Differences between the two groups were analyzed for significance by the log-rank test. Results:Among the 499 patients, the incidence of synchronous tumor was 9.4% (47/499). Logistic regression analysis showed that the lesion size ( P=0.046, OR=0.807, 95% CI: 0.653-0.996), atrophic gastritis and intestinal metaplasia ( P=0.017, OR=3.207, 95% CI: 1.229-8.371), Helicobacter pylori infection ( P=0.046, OR=1.952, 95% CI: 1.013-3.761) were independent risk factors for the occurrence of synchronous tumor after endoscopic resection. A total of 453 (90.8%)patients were successfully followed up. Kaplan-Meier curve showed that the overall survival rate of the synchronous tumor group and the non-synchronous tumor group were 95.2% and 97.6%, respectively ( P=0.72). Conclusion:The incidence of EGCC accompanied with synchronous tumor is not high, but it should not be neglected. For patients with small lesions, it is necessary to consider the possibility of main cancer in other parts. For patients with Helicobacter pylori infection or severe mucosal atrophy and intestinal metaplasia, more attention should be paid to the mucosa around the lesion during endoscopic examination and strict endoscopic follow-up should be carried out.

RESUMO

BACKGROUND: Excision repair crosscomplementing-1 (ERCC1) has been reported to play a prognostic role and may indicate the treatment response in patients with head and neck squamous carcinoma (HNSCC). Nevertheless, the strength of evidence of ERCC1 predicting these two clinical outcomes are still controversial. METHODS: Potentially eligible studies were retrieved using PubMed, Embase and Medline. Basic clinical characteristics of patients and statistical data with the survival data were collected. Then a meta-analysis model was established to investigate the correlation between over-expression of ERCC1 and survival outcome in HNSCC patients as well as to determine whether the treatment response is dependent on expression stature of ERCC1 or not. RESULTS: 17 eligible studies and 1263 patients were yielded in our meta-analysis. The pooled HRs with 95% confidence intervals (CIs) for OS and PFS were 2.14 [1.51, 3.05] and 2.60 [1.98, 3.42], respectively. In terms of subgroup analysis, race was found to be a significant factor divided for these analyses, and the pooled HRs for the Asian subgroup are 2.97 [2.05, 4.32] and 2.75 [1.82, 4.13] respectively. In non-Asian subgroup, Pooled HRs indicate the predict role for PFS 2.42 [1.60, 3.66], but no value for OS (P<0.05). With regard to treatment response, the pooled ORs were 3.04 [1.99, 4.62]. Results from subgroup analysis that divided by race further showed that pooled ORs in Asian group were 3.95 [2.30, 6.78] and 1.93 [0.97, 3.84] in non-Asian group. CONCLUSION: ERCC1 could be a fine prognostic factor of HNSCC and can also prompt the treatment response, which might be proven by further multicenter clinical trials.

OBJECTIVE:To prepare compound ofloxacin gargle, and to establish a method for its quality control.METHODS:Compound oflorxacin gargle was prepared with tinidazole, ofloxcin, heavy peppermint water, glycerol and puri-fied water.Differential spectrophotometry was employed to determine the content of tinidazole without isolation, and then the content of ofloxcin was calculated by formula on the basis of absorbability additivity principle.RESULTS:The prepared gargle was light green yellow clear liquid. The average recovery rates of tinidazole and ofloxacin were98.92%(RSD=0.35%)and99.18%(RSD=0.54%),respectively.No distinct decrease in content of these two components was found in3month test.CONCLUSION:The established method of preparation is simple, and that of content determination is convenient as well as that preparation is stable, which can be widely used in hospitals at primary level.
